What documents or forms are needed to enroll?

Enrolling your child at Playto does involve some important paperwork – but don’t worry, we will guide you through it all. Broadly, New York State licensing regulations require us to have certain documents on file for each child. These include:

  • A completed enrollment application (with your child’s basic information, family contact info, emergency contacts, etc.).

  • Medical forms: one part will be a health history you fill out, and another part is a medical report completed by your child’s pediatrician. The doctor’s form will need to include proof of an up-to-date physical exam and immunization records as required by the state.

  • If your child has an allergy, asthma, or any medical condition that needs a special care plan (for example, an EpiPen, inhaler, or medication), we’ll provide additional forms for you and the doctor to complete. These outline what we should do to manage the allergy or condition at school.

  • Various consent forms and acknowledgments: for instance, permission for us to apply sunscreen or diaper cream, emergency medical treatment consent, a media/photo release (if you’re okay with us taking photos of your child for internal use or our app), and agreements to follow certain policies (like our illness policy or code of conduct).

Most of these forms are filled out by the parents or guardians. The medical examination form is the one that requires a doctor’s signature, so it might be a good idea to schedule a pediatrician appointment if your child hasn’t had a recent check-up. Once you decide to enroll, we’ll give you an enrollment packet (either paper or electronic) with all the necessary forms and instructions. Our team is also available to answer any questions as you fill them out. We know it’s a bit of paperwork, but it’s all to ensure we have the information needed to take the very best care of your child. And remember, we keep all your information confidential and secure. After you turn in the forms, we’ll review everything to make sure it’s complete, and let you know if anything is missing before your child’s first day.
